The Northland Area Family Services Center was created as a unique way to give rural access to county and educational services, and to meet community needs through innovative programming. Incorporated in 1996 as a 501C3, we serve the Northland Community School District and parts of Leech Lake Reservation. This is a population of 4,474 people (5.26 persons per square mile). In our service area 20% of households make less than $20,000 and 11.9% or 536 people in our community live below the poverty line.

The Region V+ Mental Health Initiative serves mental health consumers in Central Minnesota. We are comprised of Aitkin, Cass, Crow Wing, Morrison, Todd, Wadena counties, and Leech Lake Band and Mille Lacs Band of Ojibwe tribal nations.
The Region V+ Mental Health Initiative Board has authority to develop and implement policy and procedures for preparation of the biennial Adult Mental Health Grant Application; to budget for the multi-county area based on the individual county and tribal nation mental health Annual Plans, the regional planning activities of the Board, and the input from consumers through the regional LAC and area mental health consumer organizations.
Additionally, the Region V+ Mental Health Initiative is responsible for the identification of adult mental health service needs and the development of mechanisms for multi-county program development, evaluation, coordination and implementation when seen as feasible, by participating counties or tribal nations; including any other mental health related programs and/or Minnesota Department of Health special project grants which come within the jurisdiction of the Board.
The Board may contract for services from private firms, non-profit corporations, state and local government agencies, sovereign tribal nations, community agencies, qualified individuals, or other sources.

- There is no “wrong door” to access services, and all Providers know how to obtain access to all services.

- All services are provided in the least restrictive environment (in compliance with the Olmstead Decision of the US Supreme Court).
- Effective support is provided at the right time and in the right place.
- Programs and services will incorporate a Holistic view of mental and physical health.
- Programs shall seek to decrease the amount and duration of involuntary services (under Commitment or duress).
- Services shall be provided in a cost effective way.

A Wellstone Family Safety Program addressing supervised visitation, safe exchange, and a violence intervention program for men. Advocates for Family Peace creates awareness in our community by promoting safety, equality, and responsibility in intimate and family relationships.
Programs offered include: Advocacy, Housing, Intervention Program for Men and Fathers, Intervention Program for Women, Women’s Empowerment Group, Wellstone Family Safety Program and Community Outreach and Education.

Northland Recovery Center offers a full continuum of chemical dependency services to include adult and adolescent inpatient and outpatient treatment using the 12 step model along with cognitive behavioral therapy. Co-occurring mental health issues are treated by on-staff licensed therapists, a psychiatrist and certified nurse practitioners. Family education and detoxification services are provided.

The Leech Lake Diabetes Clinic (LLDC) and Program specializes in the treatment and medical management of people living with diabetes and its co-morbidities, as well educating the Leech Lake community on Diabetes Prevention. Clinical appointments are offered in Cass Lake, Bemidji, Ball Club, Inger and Onigum. Clinic visits can include physical examinations, laboratory tests, diagnoses, medication prescriptions and diabetic foot exams. The medical record process is used in assisting patients achieve the standards of care for diabetes. Referrals are made as needed.
The clinic provides Diabetes Self-Management Education/Training (DSME) and Medical Nutrition Therapy (MNT) for patients, families, and friends. The information is offered in individual and group settings, classes and community events. This includes education on healthy food choices, carbohydrate counting, blood glucose self-monitoring, physical activity, weight loss, medications, insulin use, insulin pump use, smoking cessation, coping, depression, behavior change and support.

Second Harvest North Central Food Bank, a 501(c) (3) non-profit organization, serves over 100 hunger relief agencies in Aitkin, Cass, Crow Wing, Itasca, Koochiching, Kanabec and Mille Lacs counties in North Central Minnesota. We are located in Grand Rapids and administer several programs that distribute over 4 million pounds of donated and purchased food and grocery products directly to individuals and through agencies that serve the needy. The Food Bank has 15 employees, and over 500 regular volunteers who work daily or on special events.
Second Harvest North Central Food Bank supplies the most basic of human needs – Food. We provide this food to agencies that feed many hungry children, working families, disabled people and the elderly. We are experts at procuring excess food products and distributing them to hungry people. We are at the front lines in advocacy for hunger relief locally, statewide and nationally.
Second Harvest North Central Food Bank serves some of the highest poverty areas in Minnesota. An estimated 17,200 different people per month are served by the Food Bank, about 4,400 different people receive emergency food assistance in any given week. The amount of food distributed by the Food Bank rises each year as the number of people coming to our hunger relief agencies continues to increase.
