Student First: A school-based, community-driven program that helps youth identify their strengths and empowers theme to pursue their life goals. Support and guidance are provided to students by professional program staff and a one-on-one, caring adult Success Coach. Located at Bemidji Middle School, Bemidji High School and Kelliher Public Schools.

Bemidji Middle School Truancy Tracker Program: Provides mentoring, tutoring, adult role models and any other necessary means of support to youths in Grades 6-8 to help identify and address issues contributing to truancy.

Re-Engagement Case Management: Provides case management services to students with attendance issues who attend Bemidji High School, Voyageurs Expeditionary School, and Blackduck Schools to reduce truant behavior and prevent court involvement.

Truancy Case Management: Provides case management services to students and families who are involved in the Court system due to truancy.

Senior Volunteer Support Services:

Volunteers provide services such as grocery shopping assistance and local transportation for medical or other pre-approved appointments.  (We are unable to provide MA Medical Transportation)  Services are billed on a sliding fee scale.  Requests must be made 3-5 days before the need.

Homemaking and Chore Services:

Homemakers provide light housekeeping, meal preparation, laundry assistance, and shopping as needed.  Services available through county alternative care or elderly waiver, long term care insurance, and private pay basis.

Respite Care:

7 days a week 24 hours per day based on availability.  Certified nursing assistants or qualified volunteers provide relief for the primary caregivers of persons who are chronically ill or living with a disability.  There must be an identified primary caregiver to qualify for this service.  Respite care is available through county alternative care or elderly waiver, long term care insurance and also on a private pay basis on a sliding fee scale.

Powerful Tools for Caregivers:

An evidence based series of six week educational classes focusing on the needs of the caregiver, or family and friends who are caring for older adults, suffering from stroke, Alzheimer’s Parkinson’s disease or similar long term conditions.  The class provides skills and confidence to better care for yourself while caring for others.

Caregiver Coaching and Assessment:

Providing education and emotional support to caregivers.  The support group provides an opportunity for caregivers to say what they are feeing in a supportive environment while learning new strategies and resources in our community.

The primary purpose of the Caregiver Support Group is to provide education and emotional support to caregivers.  The support group provides an opportunity for caregivers to say what they are feeling in a supportive environment while learning new strategies and resources in our community.

Aging Life Care Management: 

A holistic, client-centered approach to caring for older adults or others facing ongoing health challenges.  Working with individuals, elders and families, the expertise of Aging Life Care Professionals provides the answers at a time of uncertainty through: Assessment and Service Coordination; Planning and Problem-Solving, Education and Advocacy; Family Caregiver Coaching.

ACT on Alzheimer’s:

A national program designed to create dementia friendly communities that involves: 1) Raising awareness of Alzheimer’s, 2) Transforming attitudes, 3) Moving people to action, and 4) Supporting family and friend caregivers by providing accessible information, resources, and in-person support.  There are multiple options and a variety of services and package plans.  Contact us to inquire about specific costs and to find out if you are eligible to receive a reduced rate.

CHILD CARE ASSISTANCE PROGRAM (CCAP)
Child care assistance is available to parents who are receiving assistance under the Minnesota Family Investment Program (MFIP) and parents whose income is at or below 175% of the Federal Poverty guideline and are in an approved activity. It is important for parents to know their child is well-cared for while they are at work, looking for work, obtaining training or going to school in order to secure employment. Application is required to have eligibility determined. CCAP may not cover all child care costs. Parental amounts depend on the family’s income, the number of people in the family, and how much the child care provider charges. The goal is to assist the family to maintain employment and gain self-sufficiency.

CHILD SUPPORT
Every child needs financial and emotional support and every child has the right to support from both parents. Minnesota’s child support program benefits children by enforcing parental responsibility for child support.
Either parent of a child may apply or qualify for certain support services from the Child Support Unit. Depending on the circumstances associated with the case, there may be fees assessed for collection unless the applicant receives public assistance (Minnesota Family Investment Program, Medical Assistance, MinnesotaCare, or Childcare Assistance.)

The Bemidji Area Office (BAO) provides service and support to 34 Federally-recognized Tribes and 4 Urban Indian Health programs located in Illinois, Indiana, Michigan, Minnesota and Wisconsin. Tribal Health services are provided through 11 P.L. 93-638 Title V compacts and 23 Title I contracts. Urban Indian Health programs are located in Chicago, IL; Detroit, MI; Milwaukee, WI; and Minneapolis, MN. These Federal/direct service programs, Cass Lake Hospital, Red Lake Hospital, and White Earth Health Center, are staffed by approximately 500 federal civil service employees and Public Health Service (PHS) Commissioned Officers. These federal employees are comprised of physicians, nurse practitioners, physician assistants, nurses, laboratory technicians, radiology technicians, behavioral health practitioners, dentists, dental assistants, dental hygienists, optometrists, optical technicians, dietitians, pharmacists, pharmacy technicians, physical therapists, engineers, sanitarians, equipment technicians, health information managers, administrators, acquisition, supply, finance, information technology and clerical staff. In addition, there are numerous specialty clinics provided in our Federal/direct service programs by local private health care specialists.

Tribal and Urban programs directly employ a multitude of health care providers and allied staff such as, mental health, substance abuse counselors, and community health nurses and representatives.
