Before child advocacy centers were created, child victims of crime often had to tell their stories to more than a dozen adults – police officers, prosecutors, doctors and social workers. They could be shuttled all over town giving depositions and getting medical exams. There was very little coordination between the various agencies. Consequently, children were traumatized repeatedly as they sought help. In some of the saddest cases, abused children simply fell through the cracks in the system that was supposed to save them. Likewise, prior to Sexual Assault Nurse Examiner Programs and Family Violence Intervention Programs victims were experiencing long waits in the emergency room, a lack of privacy, nervous and unskilled examiners, insensitivity, and improper evidence collection or loss of evidence.
In 2005, the Red Lake Band of Chippewa Indians, North Country Health Services, MeritCare Clinic of Bemidji, Midwest Children’s Resource Center of Children’s Hospitals and Clinics of MN – St. Paul, MN and others joined to form a 501(c)(3) non-profit organization to develop and operate a medical-model family violence center to serve the needs of Northern Minnesota.
FACNM is the only medical-model assessment center that currently exists in this region of the state. Moreover, the FACNM is one of the only centers in America to serve the needs of child sexual and physical abuse, adult sexual assault and domestic violence under one roof.
FACNM serves the entire ninth judicial district which includes the counties of Kittson, Roseau, Marshall, Pennington, Red Lake, Polk, Norman, Mahnomen, Clearwater, Lake of the Woods, Beltrami, Hubbard, Koochiching, Itasca, Cass, Crow Wing, and Aitkin. In addition, the FACNM serves Red Lake, Leech Lake, and White Earth Reservations.
Today, all of the services needed by victims and their families – forensic interviews, medical exams, referrals and family support services – are clustered around one central location. FACNM is the hub where representatives from many disciplines meet to discuss and make decisions about the investigation, assessment, treatment and prosecution of victim related cases. They work together in the best interests of the victim, providing an immediate response and ongoing support to help children and families recover from the trauma of abuse or violence.

SENIORS: We are your Aging-in-Place Specialists!

Disability Hub MN offers information about community resources related to people with disabilities, including housing, home and community-based services, and disability and cash benefits. This statewide service is a ‘no wrong door’ information and referral service for people with disabilities, their families, and support team. Disability Hub MN can be reached statewide by calling toll-free at 1-866-333-2466. The Hub is available Monday through Friday 8:30 a.m. to 5:00 p.m., to provide one-to-one assistance to help people learn about their options and connect with the supports and services they choose.

The Minnesota P.E.O. Home Fund was established in 1929 to provide charitable grants to individuals who lack financial means to provide housing, medical care, and other necessities of life for themselves. This fund primarily serves Minnesotans who may be facing unique challenges related to advanced aging, illness or disability. Charitable grants provide a bridge of support to positively impact the recipient’s life and well-being as they navigate their present struggle.
Temporary and emergency shelter for homeless families and pregnant women.
Village of Hope promotes self worth and independence through the provision of temporary shelter and supportive services for families experiencing homelessness. We are unable to house individuals without families.
Residents are selected in accordance with the following eligibility criteria:
- The family unit must be homeless as defined by HUD guidelines.
- The primary applicant must be at least 18 years old and either pregnant or have primary guardianship/custody of one or more dependent children ages 17 or younger.
- All qualified households whose income at the time of initial occupancy does not exceed 50% or greater of the statewide or area median household income as determined by HUD and adjusted for families of five or more.
- Extended family members residing with the family prior to application for shelter at Village of Hope will be included in the definition of the “family unit”.
- The applicant must not be currently abusing alcohol or drugs.
- The applicant must be stable on psychiatric medications and prescribed medications for medical conditions and be willing to take them as prescribed.
- The applicant’s criminal history will be reviewed on an individual basis. Certain offenses may result in denial of admission. These may include sexual offenses requiring the tenant to register with local officials, violent crimes against persons or property, assault, arson or destruction of property.
- The applicant must not pose a risk of serious harm to self or others.
- The applicant must express a definite and informed desire to enter the emergency shelter program, including actively participating in a housing search and case management program.
- The applicant must be willing to accept group living including:
~A tolerance and respect for other shelter residents and staff,
~A willingness to adhere to shelter rules,
~A willingness to fully participate in maintaining the shelter and sharing cleaning and maintenance duties. - If applicant is eligible for Group Residential Housing (GRH), the applicant must be willing to assist VILLAGE OF HOPE in verifying the requirements including documentation of:
~Disability and/or chemical dependency,
~Income and/or asset verification confirming applicant fits the requirements of the program.
Intake Procedures
Clients or referents call the central intake number (218-751-0722). General questions are answered regarding eligibility, homelessness, and anticipated openings. Referents that are not able to be accommodated immediately are placed on an optional waiting list while every attempt is made to assist the referent in identifying an alternative shelter plan.
Upon vacancy of a family unit the waiting list is reviewed and families with the highest level of need are invited to participate in the application process. This process includes the opportunity to:
1. View the facility
2. Review the terms of the agreement to participate in a housing search and case management
3. Review the resident rules
4. Sign releases of information to verify homelessness
5. Sign releases of information to verify income
Homelessness is determined by utilizing a checklist for eligibility and receiving the verification by a third party in writing, on the Minnesota Housing ”Housing Status Verification/Referral Form” whenever possible. In cases where no written independent verification is possible from a shelter, a case manager or other 3rd party, clients must complete and sign the Minnesota Housing “Housing Status Self-Certification Form” .
Once homelessness is verified, in writing, on the forms provided by Village of Hope, and exclusionary criteria are determined not applicable, applicants are forwarded to the Executive Director for review.
If an applicant is approved, they are accepted and will be allowed to move into Village of Hope. Once a room is available, the homeless advocates will coordinate a move-in date and time with the applicant.
If you or your family lives in any of the following situations: doubled up with other people due to loss of housing or economic hardship, living in cars, parks, public spaces, or abandoned buildings, at imminent risk of losing housing (foreclosure, eviction, etc.), substandard housing (lack of electricity, lack of plumbing, insufficient heating, or safety concerns), living in hotels, motels, trailer parks, camping grounds due to lack of adequate accommodations, or are living in shelters or transitional housing, you may be eligible for the following services. The mission of the Epilepsy Foundation is to lead the fight to overcome the challenges of living with epilepsy and to accelerate therapies to stop seizures, find cures, and save lives. For more than five decades, the Epilepsy Foundation and our network of nearly 50 partners have helped to:

For nearly 40 years we have been advocating for and serving the LGBTQ+ community, those living with HIV, and all folks facing barriers to equitable healthcare. Since our formation through the Minnesota AIDS Project and Rainbow Health Initiative merger—and later the acquisition of Training to Serve—we have continued to grow and identify new ways to commit to our mission.

- We help people navigate healthcare systems by breaking down barriers, particularly LGBTQ+ people and those affected by HIV.
- We work to End HIV by building on our four-decade legacy of providing the most comprehensive array of Minnesota AIDS Project’s wrap-around services for people with HIV. As a disease of injustice, the only way to end HIV is to end health disparities.
- We offer unique and specialized mental and chemical health services by and for the communities we serve. Our care is trauma-informed, harm-reduction based, sex-positive, and meets people where they are. We turn no one away for inability to pay.
- We work to empower people on the margins with relevant information, support, and connection to resources that allow them to lead healthier lives. Our legacy of four decades fighting HIV and LGBTQ+ health disparities has given us insight and experience that we can bring to the broader movement for health equity.
- We educate and train providers about how to deliver better care for the diversity of LGBTQ+ and HIV communities through a lens of intersectionality.

Support and services for victims who are or have experienced domestic and sexual assault, dating violence and stalking. Services area is the Leech Lake Reservation including portions of Cass, Hubbard, Itasca and Beltrami Counties.

The Office of Ombudsman for Long-Term Care is a program of the Minnesota Board on Aging. Regional ombudsmen and volunteers work to enhance the quality of life and services for people receiving long-term services and supports. The program also advocates for reform in long-term care through changes in state law, federal law and administrative policy.

An Ombudsman is an independent consumer advocate who:
- Investigates complaints about the health, safety, welfare and rights of Minnesotans receiving long-term services and supports
- Works to identify problems and resolve individual concerns
- Provides information and help with long-term care services, consumer rights and regulations
- Resolves disputes between consumers and providers about long-term care services
- Works with providers to promote a culture in which people have and can make choices.
Ombudsmen can help:
- Residents of nursing homes and board and care homes, including veterans’ homes
- Residents of adult care homes, such as housing with services, assisted living, customized living or foster care
- People receiving home care services
- Medicare beneficiaries who have concerns about getting into or being discharged from hospitals
- Anyone seeking help with long-term services and supports.
Ombudsmen work with residents, families and service providers to promote person-centered care and to identify issues in the long-term care system and advocate for change. They also handle complaints and problems from individuals related to:
- Quality of care and quality of life
- The Patient, Resident and Home Care Bill of Rights
- Discharge or eviction from nursing homes, board and care homes, veterans’ homes, assisted living and other long-term care or home and community-based service settings
- Termination of services, including home care, adult foster care, hospice, Elderly Waiver, Community Access for Disability Inclusion waiver and other long-term care community-based service programs
- Public benefit programs, such as Medicare, Medical Assistance, veterans’ services, long-term care insurance and other programs that directly affect an individual’s long-term care needs.
An Ombudsman is an independent government official responsible for reviewing government and government regulated agencies in an effort to ensure that their practices are fair, reasonable and appropriate. To do this, the ombudsman: Receives complaints, reviews, investigates and if appropriate, makes recommendations to remedy the complaints.

The Northland Area Family Services Center was created as a unique way to give rural access to county and educational services, and to meet community needs through innovative programming. Incorporated in 1996 as a 501C3, we serve the Northland Community School District and parts of Leech Lake Reservation. This is a population of 4,474 people (5.26 persons per square mile). In our service area 20% of households make less than $20,000 and 11.9% or 536 people in our community live below the poverty line.
SERVICES PROVIDED: Rule 25 assessments, classroom curriculum, Red Cliff, Protecting you, protecting me. Family prevention with the traditional based program, Bii-zin-da-de-dah. Youth recreational activities, also community based prevention activities.
Community Resource Connections (CRC) is a nonprofit membership organization serving Beltrami, Cass, Clearwater and Hubbard Counties.
MNsure Navigators: Application for Medical Assistance, Minnesota Care, and Qualified Health Plans offered through the state of Minnesota’s health exchange. CRC staff provide computer and internet access, and question by question guidance. No cost to you. Appointments required.
Service Access: Assistance finding the resources you need. CRC staff will educate you about your service options, assist you with service applications, and advocate for you when the system doesn’t work as you need it to work. No cost to you.
Nutrition Assistance Program for Seniors (NAPS): CRC’s Blackduck office site takes applications and serves as a dissemination point for food commodities provided through North Country Food Bank. Eligibility is based on age (60+) and income (call for guidelines). Free monthly food boxes containing a $40 value of non-perishable food items.
Community Service Agency Membership: CRC works with community service agencies to organize and publish a hard copy resource directory as well as this electronic version. Other member services include: facilitation of networking meetings, cooperative marketing events, nonprofit start up guidance, and program support.

BACKPACK BUDDIES
In the Bemidji area, 1 in 5 children experience food insecurity and hunger. The Backpack Buddies program was developed to help ensure that children in the Bemidji school district have access to nutritious, non-perishable and easy-to-prepare food at times when other resources are not available, such as weekends and school vacations.
The program provides backpacks filled with food that is child friendly, non-perishable, and easily consumed free of charge. An average weekly menu consists of: 2 entrees, 1 snack, 2 breakfast items, 1 vegetable, 2 fruits, and 2 milks, and 1 high protein item. The food is purchased from the North Country Food Bank and community groups and volunteers package the food each month.
The food packs are discretely distributed to children, as best determined by the school, on the last day before weekends and all scheduled school breaks/holiday breaks.
Thanks to Sanford Health, the program has expanded to feed more than 450 children, who have been referred by school staff as experiencing food insecurity or hunger, each weekend and school break. The program currently serves all Bemidji school district schools.
COATS FOR THE COMMUNITY
Each year donations of gently used, clean winter coats and outerwear for men, women, children and infants are collected. More than 3,000 cold-weather items are collected and distributed to our Bemidji neighbors to keep them warm and dry during the winter.
HOLIDAY GIFTS FOR KIDS
Each year between 800 and 1000 children, ages 0-16, experience a joyful Christmas thanks to the generosity and kindness of others. Parents or legal guardians register for the Holiday Gifts for Kids program and have the opportunity to choose a gift for their child on distribution day. In addition, each family receives a board game to enjoy together.

We are established nonprofit service providers and caregivers in your community. We are NOT county, state or federal employees, nor do we work for health management systems like Prime West Health. We are reimbursed through contracts and grants that enable us to serve you without charging for that service.
We are trained and certified by MNsure to assist with online application for health insurance in Minnesota. The online application is faster and more reliable than paper applications submitted to the county system.
We work cooperatively with your County and State Human Service Departments, Minnesota Care, and MNsure. We serve as your advocate and guide. We also work with area MNsure Brokers who can help you choose the best Qualified Health Plan; if that is what you qualify for through the MNsure application system.

Resident involvement in all aspects of daily living is encouraged. With our staff providing guidance, residents take part in personal grooming, meal preparation, and household tasks. Resident activities are offered three times daily and adapted for individual residents. |

All services are open to adults 18 years old or older who have a long-term severe and persistent mental illness. Services include Adult Rehabilitation, Mental Health Services (ARMHS). Outreach, diagnostic assessments, medication reviews with a psychiatrist, after-hours emergency help, nurse visits, community/public education, phone contact, social and recreational activities in a drop-in center and off-site client employment help, and the REACH family support group.

Enrollment – Schoolcraft Learning Community (SLC) enrolls 178 students in grades kindergarten through 8. Because of our small size, we are able to maintain a caring community for our learners.
Class Size and Structure – Average of 18 students in all grade levels. Grades 2-7 are multi-aged.
Faculty – More than 30 men and women; all of whom are highly qualified.
Student-to-Faculty Ratio – Averages 8:1 schoolwide.
Student Life – The primary method of instruction at SLC is through learning expeditions. Learning expeditions are long-term, indepth investigations of a theme or topic that engage students through authentic research, projects, fieldwork, service, and adventure. Using the integrated approach, students receive instruction in French, music, piano in grades K and 1, art, physical education, and conflict management. Orchestra is also available for grades 2-8.
Meal Program – All students are offered a balanced breakfast and a delicious lunch each day. Students and staff are treated to many different ethnic varieties of foods. Schoolcraft Learning Community does participate in the free and reduced lunch program. Students may elect to bring lunches from home. Check out our menu!
Campus & Facilities – SLC leases Lac du Bois (the French village) from Concordia Language Villages during the school year. The transition from summer camp cabins to classrooms is almost magical. We have 10 cabins/classrooms spread out over two smaller campuses, separated by a soccer field. We have access to lots of northern Minnesota outdoors as an important part of our classroom environment.
Disability Hub MN offers information about community resources related to people with disabilities, including housing, home and community-based services, and disability and cash benefits. This statewide service is a ‘no wrong door’ information and referral service for people with disabilities, their families, and support team. Life Wireless is a Lifeline supported program that provides free cellular service. Only eligible consumers may enroll in the program and documentation is required. Life Wireless also provides plans for people who do not qualify for assistance. Benefits include: voicemail, text messaging, caller ID service, nationwide calling, and more.
