Minnesota’s Personal Care Assistance program is designed to support people of all ages with disabilities to live independently in the community. Personal Care Assistants (PCAs) provide services to people who need help with activities of daily living, instrumental activities of daily living and health-related functions. PCAs also provide hands on assistance, supervision, redirection and intervention for behavior including observation and monitoring.
A PCA may assist the person with the following ADLs:
- Dressing – Application of clothing and special appliances or wraps.
- Grooming – Basic hair care, oral cares, shaving, basic nail care, applying cosmetics and deodorant, care of eyeglasses and hearing aids.
- Bathing – Basic personal hygiene and skin care.
- Eating – Completing the process of eating including application of orthotics required for eating, hand washing and transfers.
- Transfers – Assistance to transfer the person from one seating or reclining area to another.
- Mobility – Assistance with ambulation.
- Positioning – Assistance with positioning or turning a person for necessary care and comfort.
- Toileting – Helping person with bowel or bladder elimination and care. This includes transfers, mobility, positioning, feminine hygiene, use of toileting equipment or supplies, cleansing, inspection of the skin and adjusting clothing.
A PCA may assist an adult recipient with the following:
- Accompany to medical appointments
- Accompany to participate in the community
- Assist with paying bills
- Communicate by telephone and other media
- Complete household tasks integral to the PCA services
- Plan and prepare meals
- Shop for food, clothing and other essential items
Under state law, a licensed health care professional may delegate health-related procedures and tasks to a personal care assistant. Examples of health-related procedures and tasks include:
- Assistance with self-administered medications
- Interventions for seizure disorders, including monitoring and observation
- Range of motion to maintain a person’s strength and muscle functioning
A PCA service may include observation of a person who:
- Has episodes of behaviors
- Needs redirection to remain safe in his/her environment
The PCA program provides support to:
- People with disabilities of all ages
- Elderly (over 65 years)
- People with chronic health conditions

A faith-based community of people who assist individuals and families in the surrounding communities by providing a facility to house services that meet spiritual, emotional, physical and education of the people. Services include, but are not limited to: Adult Day Services for seniors, After School programming for youth, Recovery support, CD Support, Grief Support, CD and mental health counseling and coaching, Anger Management, and access to a local thrift shop.

The Garden Place is an assisted living facility in Bagley, MN. The Garden Place offers activities at their location for residents. These activities generally allow residents to maintain healthy lifestyles by encouraging movement and socializing with their peers.
Being able to chat with other residents becomes an important part of many peoples’ lives and The Garden Place offers common spaces indoors to support that need. The benefit of living in an assisted living community is that making meals can be costly and time consuming process so The Garden Place provides meals for residents.
Staff is awake and available 24 hours a day so if any emergencies occur no matter the time, there will be someone ready to help. Making sure residents with diabetes monitor their insulin levels is clearly an important task and The Garden Place can help with that task. If a resident needs assistance moving from a bed to a wheelchair, this facility has staff who can help.

The Clearwater Day Activity Center focuses on each individual’s needs to develop a specialized program in the areas of Home Living, Community Integration, Employment, and Independence. With one-on-one and group-centered training, those that attend the Clearwater DAC become more independent in daily living skills, are better able to contribute and interact within their community, and build lasting relationships through outside employment and group activities.
The participants at the Clearwater DAC enjoy a variety of activities from shopping, picnics at the park, bowling, and Special Olympics to family outings and real-life jobs within the area. Each activity promotes their individuality and the DAC’s mission to see these persons succeed on a daily basis with the skills they learn to thrive in every day life.

Training and support for adults with developmental disabilities in all areas of living.

Sanford Rehabilitation is a leading provider of acute inpatient rehabilitation throughout the region. For more than 20 years, we have provided services to help individuals attain a higher level of self-sufficiency and minimize the human and economic cost of illness and injury. We are accredited by CARF, the Commission on Accreditation for Rehabilitation Facilities for our Hospital Inpatient Rehabilitation Programs. In addition, we have specialty accreditations in Fargo for acquired brain injury rehabilitation and in Sioux Falls for stroke rehabilitation. Both Fargo and Sioux Falls inpatient rehabilitation programs serve adolescents and adults that have temporary, progressive or permanent illness or disability. We take a team approach and work together to help each patient achieve maximum functioning.

The Bagley Senior Center’s activities include: Meals on Wheels, Senior Dining, Consignment Craft Room, Thrift Store, Weight Watchers, (Wednesdays at 5:30) Nutrition Assistance Program for Seniors, Yarn Group (1st and 3rd Tuesday from 12:45pm – 2:00pm) and the last Saturday of the month breakfast fundraiser (8:00am – 11:00am)
