Assessment
White Earth Substance Abuse will only be providing same day assessment appointments.
Assessments are available to enrolled members, descendants and non-natives. They are free if they live within the boundaries of the White Earth Reservation. Non-native assessments are subject to a fee.
Clients can begin calling the Substance Abuse office starting at 8:00 am the day they would like to have their assessment done.
White Earth Substance Abuse Program has the right to refuse services at any time during an assessment. If needed, you may be asked to leave or reschedule.
Prevention Program
Our prevention program performs community and school education on the dangers of drugs and alcohol with the aide of visual presentations, hands on exhibits and health fair booths on the various forms of substance abuse. They also provide information on how to contact our program for necessary services.

The mission of the Oshki Manidoo “New Spirit” Center is to be a healing place with a holistic approach for Native American youth and non-Native American Youth to begin to embrace healthy behaviors and attitudes that will transform them, their families and communities.
Program emphasis includes substance use disorder, cultural foundations, physical wellness and strengthening family connections.
Youth will return to their community following successful discharge with a new outlook, understanding their importance as an integral part of the larger community, chemical free and able to sustain sobriety and continue their journey toward healthy living.

Comprehensive Health
The Red Lake Comprehensive Health provides the management, leadership and direction needed to provide quality health services to the Red Lake people. Our main goal is to elevate the health and health care of our Red Lake Tribal members and their descendants to the highest possible level.
Emergency Medical Services
Two Ambulance Services, one in Red Lake and one in Ponemah provide prompt emergency medical care for sudden injury or acute illness and immediate transportation to a medical facility where further care can be rendered to eliminate severe complications from untreated trauma, twenty-four hours a day/seven days a week.
The Ambulance Service are licensed by the State of Minnesota as a Basic Life Support Service.
The Ambulance staff are required to be certified State Emergency Medical Technicians in order to maintain our Basic Life Support (BLS) License. The majority of our Emergency Medical Technicians are Nationally Registered Emergency Medical Technicians.
The Emergency Medical Technicians provide in-services and training to tribal members that want to learn Cardiopulmonary Resuscitation, First Aid and Emergency Medical Technician skills. We are licensed by the State of Minnesota as an official training program for Emergency Medical Technicians, CPR & First Aid.

Medical Transportation Services
Medical Transportation is provided for the people to the Red Lake Hospital, Ponemah Clinic and medical facilities off the reservation.
Patients are transported to Minneapolis, Rochester, Duluth, St. Cloud, Minnesota, Fargo, Grand Forks, North Dakota and other towns as directed by the Medical Staff. Medical transportation is provided to the Red Lake Hospital, Ponemah Clinic and to the Bemidji Hospital and Clinics on a daily basis.

Noon Meals are delivered to home bound elderly and handicapped residents in Ponemah Monday through Friday.

Community Health Nursing Services
Community Health Nursing Services provides health services to meet the assessed health needs of individuals and families in our four communities. Services are provided in hospital, clinic, home, school and community settings.
Community Health Nursing Services are provided within the context of total health care, including physical, social, psychosocial, environmental developmental, parenting education, nutritional economics and functional health. Nursing is provided at three levels of prevention: The primary prevention level focuses on health promotion and prevention against diseases, the secondary prevention level focuses on early diagnosis and prompt intervention in the disease process, and the third prevention level focuses on the use of rehabilitation activities to prevent further complications and to restore as much as possible optimal functioning. Language, communications and cultural considerations that influence care are considered. Individuals, families, and communities are encouraged to be active participants in determining their needs and reaching health outcome goals.

Ponemah Health Services
The Ponemah Health Staff provide direct quality health care to the Ponemah residents in the Ponemah Clinic & the community. The health services include Maternal and Child Care Services, Chronic Illness Care, Acute Minor and Common Illness Care, Health Education/Disease Prevention and daily clinics.
The Ponemah Nurse Practitioner and her assistant provide Women’s Health Care Clinic once a week at the Red Lake Hospital.

The Physicians provide medical services to all eligible recipients in accordance with the standards and protocols of the Red Lake Hospital.
The Emergency Room Physicians provide emergency and urgent care services 24 hours a day 365 days a year, to all eligible recipients in accordance with the standards and procedures of the Red Lake Hospital.

Physical and Occupational Therapy Services
Physical and Occupational Therapy serve a variety of disease and disabilities occurring in all ages. The Physical Therapist provides services for neurological disorders, arthritic conditions, persons with fractures, strokes, burns and numerous physiological and musculoskeletal conditions.

The Comprehensive Health Services supplementes the Indian Health Service with needed personnel in order to maintain the highest possible continuity of medical care for our people. Dietary Services provide nutrition and dietetic services to individual patients, families and communities through a variety of methods including meal service, counseling, education, community services, and in-service consultation in the hospital.

Nutrition Services
Nutrition services provides and promotes sound nutritional practices for individuals, families and groups in the Red Lake Hospital and Health Care Facility, Ponemah Health Center, Elderly Nutrition sites, Maternal Child Day Care Center and the Headstart Programs; preventing potential health problems; and providing clinical nutritional and diabetic services for patients with existing medical problems.
Nutritional Services integrated with medical and interdisciplinary support services in outpatient clinics, health centers, community health clinics, school clinics and Women, Infants and Children (WIC) services.
Women, Infants and Children (WIC) helps families eat well and stay healthy. The WIC program provides nutrition education, healthy foods and health services. WIC does nutrition assessments, and provides one-on-one counseling about food, nutrition, and breastfeeding to help you feed your family in a healthy way. WIC gives you vouchers to buy healthy foods. WIC helps you with referrals to other health and social services. To qualify for WIC you need to have a nutritional and/or medical need, and meet WIC income guidelines.

Environmental Health Services
The Sanitarian assists all communities in improving the public health. Environmental Health Services identifies and helps correct environmental conditions that are detrimental to the people’s health status. Security Services
Security services are provided for patients, staff and property at the Red Lake Hospital and Health Care Facility and the Extended Care Center. Security services work cooperatively with the local law enforcement agency for added safety.

Mental Health/Social Services
The Comprehensive Health Services provides two Independent Licensed Social Workers, Social Workers and School Wellness Counselors to assist individuals, patients, families in the schools and community promoting positive behavior, health promotion and disease prevention through direct assistance, counseling and coordination between home, school, and community and health facility.
The staff provide outreach services and function as a community resource in dealing with maladaptive behavior, and provide intervention in seeking out and eradicating conditions that caused or contributed to mental disorders.
The School Wellness Counselors provide counseling in the schools and communities.
The staff work with individuals who are having thoughts of hurting themselves, are having suicidal ideation, self-injurious behavior and behavioral emergencies.

Maternal Child Day Care Services
The Day Care staff provide services to eligible Indian children in need of a controlled quality health care environment that provides a safe place for emotional security and social development, where personal hygiene skills, proper eating habits and nutrition are taught.

The Adult Dental Program
Comprehensive Health Provides $25,000 a year to purchase dentures and partials for tribal members and their descendants at the Red Lake Hospital only.
Optometry Services
Optometry provides comprehensive optometric services including vision assessment, eye evaluation, retinal assessment, frame adjustment and repair, eye injury treatment, eye disease treatment, and contact lens fitting.

Special Diabetes Program for Indians – SDPI
The Red Lake Diabetes Program provides excellent care and education to all patients with diabetes, prediabetes, and those at risk for diabetes. Patients are seen as outpatients, inpatients and in the community. We provide one on one diabetes education, nutrition education, physical activity information, community diabetes screenings, youth wellness summer camps, diabetic socks and testing supplies given, current information on diabetes is provided to patients and diabetes related information is submitted to the Tribal Newsletter on a quarterly basis.

Ship Program (Statewide Health Improvement Program)
Provides healthy lifestyle services to promote physical activity, reduce obesity and deter smoking and tobacco use in our youth.
Nursing Home Services
Nursing Home Services are provided at the Jourdain/Perpich Extended Care Center which is attached to the Red Lake Hospital and Health Care Facility. We have 47 beds available to serve our elders and handicapped individuals. All residents served are federally recognized Indians or descendants of federally recognized tribes.

Housing Authority Programs
Rental Programs:
Low Rent Program and NAHASDA
This program is to provide families, with low incomes, affordable housing. It offers a wide variety of single-family homes (2, 3, 4, and 5 Bedroom), duplex and fourplex to qualified individuals on a month-to-month lease basis. These units are located in all four reservation districts.
ARRA Rental and Marif Rental
The programs are for low rent income families. The units are all single family rental units and located in the Ponemah district.
Federal Tax Credit
The program is for families who are at or below 50% of the area median incomes. The units are single family and duplex units and are located in all four districts.
Homeownership Programs:
Mutual Help, NAHASDA and Walking Shield
The intent of the programs are to provide families, with low incomes, an opportunity to own a home rather than rent a home. The Housing Authority shall continue to offer qualified individuals, a lease with an option to buy; any of the remaining single family homes in the mutual help inventory. as they become available. The units are all single family units and are located in all four districts.
Federal Tax Credit
To obtain a home under this program, the homebuyer must be low income and agreeable to a mandatory fifteen year lease-to-own status. The first fifteen years are considered rental. At the end of that fifteen year period, the tenants will be offered the opportunity to continue their month-to-month lease or buy out the balance of the mortgage on the home. The units are single family and are located in all four districts.
Modernization Assistance
A one-time limited modernization assistance is offered to elders age 62 or older and handicapped individuals for medical needs documented by a physician or work necessary to keep the unit structurally sound. This work cannot exceed $8,000.00. This includes material, labor and insurance.
Beltrami County outside the Reservation Rental Assistance
This program is to assist low income families an opportunity to finding housing in the private sector. Assistance to be provided will be the first and last months’ rent and the security deposit. The maximum of assistance to be provided will be $2, 100.00.
Beltrami County outside the Reservation Down Payment and Closing Cost Assistance
This program is to assist families who have an income of not greater than 80% of area median income with down payment and closing cost assistance. The amount of assistance varies from approximately $15,000.00 to $25,000.00.

Housing Finance Corporation Programs
Mortgage Program
Home loans up to $120,000.00 at 5% interest for 30 years. Renovation loans up to $35,000.00 at 6% interest for 15 years.
Rental Program
Units which have had the title turned back to the corporation are offered as rental at a set rental rate. The units are single family and are located in all four districts. The units are available for purchase from the corporation.

The Red Lake Homeless Shelter provides emergency shelter to families and single adults over the age of 18 inside the homeless shelter providing beds, meals, showers and transit transportation with case management to look for housing and other resources residents may be eligible to receive.
We have different grants with the state of Minnesota that allow us to provide rental subsidies where we pay up to 70% of a clients rent. We have 14 rental subsidies with the Minnesota Housing and Finance Agency (MHFA) and these subsidies can last up to five years. We have 5 with the Transitional Housing Program (THP) and these subsidies can last up to two years.
We have a Long Term Homelessness (LTH) grant that helps clients who qualify get housed with a security or utility deposit. This grant would also help with birth certificates, drivers license or background check fee only to secure housing. We also have a Family Homeless Prevention Assistance Program (FHPAP) grant where we could help with back rent and security deposits to prevent client’s from homelessness. We also can pay for a first or last month’s rent, security deposit if they are already homeless to get them housed. This grant has very little funding so we don’t use it as often. We have a Minnesota Assistance Council for Veterans (MACV) grant that helps veterans and their families with various needs.
The eligibility criteria is that a person must be an enrolled member of a federally recognized Indian Tribe and also be 18 years of age for all services except the Student Work Orientation service, which is 16 years of age or older.
We have a HUD Section 811 program that we call the Red Lake Supportive Housing 1rental program. This housing includes 14 units built in 7 duplexes, some of which are located in each of the four communities on the Red Lake Indian Reservation. All units will be restricted to disabled households. A disabled household means a household composed of one or more persons, at least one of whom is an adult (18 years or older) who has a significant disability. For purposes of this housing, eligible disabilities include those that are physical, mental health, or developmental in nature.
The Red Lake Nation Head Start serves 160 children on the reservation. We have two sites. One is located in the Early Childhood Center and one is located in Ponemah.

Red Lake Elementary School (RLES) and the Early Childhood Center (ECC) is an indescribably amazing and important place! Within these two buildings, over 700 precious young lives spend a good portion of their day! With that in mind, it is very, very important that your child loves school! Truly, there is very little that is more essential than for a young person to have a rich and enjoyable school experience.
Community Education
Community Education at the Red Lake School District #38 provides a Holistic Approach to educational based programming, activities and events. The best interest of the reservation community includes finding ways to utilize the funding which will be inclusive of residents of all ages.
Community Education Youth Leadership
The Leadership portion of Community Education gives students the opportunity to develop skills which will apply to life skills. This includes time management, cooperation in groups, volunteering for projects which enhance the reservation community, job skills and a connection to education through good school attendance.
Students are recruited to assist with the projects that Comedu offers such as the Teen Health Fair, Comprehensive Health Fair, sports events, educational camps, field trips, concessions at the Red Lake High School and collaborative efforts with other programs.
Youth Enrichment
The theory of Best Practice in the educational system means that students are able to learn and enrich their horizons by hands on learning. Along with hands on learning we recognize that Native youth have certain learning styles. As each student has a learning style which may be different, acknowledging and honoring the child and youth enriches their perspective of the educational system. Collaborations with Targeted Services, Home School Liaisons, Comprehensive Health,Chemical Health, Indian Health Services,Oshkiimaajitadah and other programs gives Community Education the ability to provide services to students.
Enrichment projects are also Cultural based, Native American, primarily Ojibwe. There have been several films made by students along with drum making, beading, craft designs. Youth have been encouraged in projects such as Targeted
Service to increase knowledge through hands on learning.
Kloster Park
Kloster Park is a summer baseball/softball program based in Clearbrook, Mn. Both boys and girls 9-13 years of age are eligible to attend. This give students one day per week for eight weeks to meet other youth from the surrounding area. There is
a World Series at Clearbrook which brings together families and numerous communities.
Students who have participated at Kloster Park are eligible to become coaches and/or umpires when they turn fifteen. This is a great opportunity for youth to use this coaching time on their resumes.
Teen Health Fair
Each year on the first Friday of November, Community Education along with numbers of partnerships/collaborators hold a Teen Health Fair at the Red Lake High School.
The Teen Health Fair, an annual event hosts a Holistic approach
to student’s cultural background. There is a host drum which is in the middle of the gym and a variety of disciplines.
Indian Health Services, Comprehensive Health Services, Evergreen House, Chemical Health, Early Childhood Family Education, Red Lake Tribal College, massage therapy, biofeedback, herbal therapy, aroma therapy, diabetes information and numbers of other booths are available as resources and education for students.
Parents As Teachers
Community Education is a partner with the Parents As Teachers program. Parents As Teachers is a curriculum that gives parents the opportunity to learn about being their children’s first teachers. The classes are geared for Early Childhood but can be used for children of any age.
Classes are offered at the Early Childhood center at Red Lake and at Oshkimaajiitahdaa.
Workshops
Community Education has hosted numbers of workshops with themes on Parent Education, Prevention of birth defects, prevention of fetal alcohol syndrome, Parents As Teachers and other related topics. It is the intention of the Red Lake School district to provide educational outreach for the families and reservation community.
55 and over Insurance classes
It is our intention to hold the 55+ classes each year. There are the regular classes and the renewal classes. We have not been able to secure an instructor this past year but are working to continue this service.
Crafts Classes
Crafts classes are held in Ponemah at the Boy’s and Girl’s club. The classes are for adults but the environment at the club helps so that participants can bring children who can then have their own activities. Classes will be blanket making, pillow making and other crafts. Christmas there is cookie making and an assortment of fun give away presents. LIVIT
Community Education is a partner with the Department of Indian Works, Minneapolis which has provided us with a curriculum to teach Sexual Health. Workshops are offered in the Red Lake High School and Oshiimaajitahdaa.
Summer Activities
Summer activities for youth are planned with a number of partners which also include Red Lake Law Enforcement. It is the intent of the district to assist in planning a continuum for a variety of activities during the summer.
Community Education Advisory Board
The Community Education Advisory Board has four meetings per year. The board is a volunteer group and discusses what resources are available for the reservation community.
NWICDC services imbed Indigenous worldviews and values with Western methodology to braid concepts of inclusion, equity, opportunity, wellness, and self-sufficiency to advance a just and supportive society. Inclusion ensures that all individuals have equitable access and involvement in all aspects of community and decision making.
Equity focuses on ensuring fair and just allocation of resources for individuals to have the things they need to be successful in life. Opportunity is the equitable access for all individuals to participate and be represented in all aspects of society. Wellness extends beyond physical health to encompass mental, emotional and spiritual well-being, vital for individuals to thrive. The Minnesota Chippewa Tribe, comprised of the Bois Forte, Fond du Lac, Grand Portage, Leech Lake, Mille Lacs, and White Earth reservations, is a federally recognized tribal government that, through unified leadership, promotes and protects the member Bands while providing quality services and technical assistance to the reservation governments and tribal people.
Creation of the Constitution
MCT was created in order to form a united Chippewa Tribal organization whose purpose and function is to conserve and develop Tribal resources and common property that would promote the general welfare of its members and future generations. This required the Ojibwe Bands of Minnesota, and other Tribal Nations in Minnesota, to conform to the Indian Reorganization Act (IRA) in 1934 in order to take advantage of the economic and social development programs proposed by the United States.

Title III. Congregate Meals
Meal provision to an elderly client or other eligible participant at a nutrition site, senior center or some other congregate setting.
Eligibility Requirements
Persons who are 60+ years of age or older and spouse, regardless of age and other eligible individuals.

None, contributions are accepted.
Guests can eat for the cost of $4.50.
Title III. Home Delivered Meals
Meal provision to an elder or other eligible participant at the client’s place of residence.
Eligibility Requriements
Home delivered meal recipients must be 60 years of age or older, home bound by reason of illness, disability or functional need, and unable to prepare their own meals. Meals may be made available to individuals with disabilities who reside in a non-institutional household with and are accompanied by an individual eligible for congregate meals.

Title VI. Congregate and Home Delivered Meals
Eligibility will include enrolled members of any recognized tribe aged 55-59 years or over within the program service area. Eligibility will include spouse, handicapped or disabled individual in the same household of the eligible participant who accompany participant to a congregate site.

The Cass Lake Indian Health Service Unit is owned and operated by the Federal Government as a Public Health Service, Indian Health Service Facility. The Service Unit has an active user population of 12,000 clients and provides over 200 full-time jobs in the community.
The Service Unit operates a Critical Access Hospital (CAH) and Outpatient Ambulatory Care Clinic that offers Medical, Dental, Pharmacy, Laboratory, Radiology, Rehabilitation Services, Audiology, Podiatry, and Optometry Services. Cass Lake IHS also provides 24/7 access to Emergency Care services.
The Leech Lake Band of Ojibwe manages the Behavioral Health programs, Substance Abuse programs, a Diabetes Center and Public Health Nursing at six community based clinics on the reservation.

The Bemidji Area Office (BAO) provides service and support to 34 Federally-recognized Tribes and 4 Urban Indian Health programs located in Illinois, Indiana, Michigan, Minnesota and Wisconsin. Tribal Health services are provided through 11 P.L. 93-638 Title V compacts and 23 Title I contracts. Urban Indian Health programs are located in Chicago, IL; Detroit, MI; Milwaukee, WI; and Minneapolis, MN. Tribes in the Bemidji Area include Ojibwe (Chippewa), Ho-Chunk, Menominee, Mohican, Oneida, Odawa, Potawatomi, and Sioux.
The BAO operates 3 Federal/direct service programs on behalf of the Leech Lake Band of Ojibwe, Red Lake Band of Chippewa Indians and the White Earth Band of Chippewa Indians of Minnesota. These Federal/direct service programs, Cass Lake Hospital, Red Lake Hospital, and White Earth Health Center, are staffed by approximately 500 federal civil service employees and Public Health Service (PHS) Commissioned Officers. These federal employees are comprised of physicians, nurse practitioners, physician assistants, nurses, laboratory technicians, radiology technicians, behavioral health practitioners, dentists, dental assistants, dental hygienists, optometrists, optical technicians, dietitians, pharmacists, pharmacy technicians, physical therapists, engineers, sanitarians, equipment technicians, health information managers, administrators, acquisition, supply, finance, information technology and clerical staff. In addition, there are numerous specialty clinics provided in our Federal/direct service programs by local private health care specialists.
Tribal and Urban programs directly employ a multitude of health care providers and allied staff such as, mental health, substance abuse counselors, and community health nurses and representatives. Services not available through Tribal or BAO Federal/direct service programs may be delivered through Contract Health Services (CHS).
Our mission is to provide a quality and creative approach to individual and community based exercise programming to ultimately control and prevent diabetes in our community.
Also note: person(s) do not need to have Diabetes to use the fitness center.

Gym Rules
- Diabetic clients have first priority on the equipment
- Use equipment at your own risk
- Sanitize equipment after use
- Return all equipment after use and re-rack your weights
- No horseplay allowed
- Appropriate footwear required while using equipment and lifting weights
- Know your limits; use a spotter as needed
- No gym bags, coats, etc. on gym floor; use lockers and coat rack
- Not responsible for lost or stolen items; locks available upon request
- No food allowed on the gym floor
- All liquids must be in containers with lids
- For safety reasons, no children under 10 allowed
- Be considerate of others
Code of Behavior
All fitness center participants are required to conduct themselves in an appropriate manner. Inappropriate language (swearing, derogatory terms, etc.), inappropriate behavior and harassment are not allowed on the premises. Proper use of the equipment is required. If you do not know how to operate the machines, the fitness staff will be happy to assist you. There is a zero tolerance policy when it comes to horseplay in the fitness center and on the machines. You will be asked to leave if you do not follow the Code of Behavior and Gym Rules!
Age Restrictions
- Children 13 and under must have an adult with to supervise them during each visit.
- Children 11 and under must have a medical clearance on file.
- For safety reasons, no children under 10 allowed.
- Young adults 14-17, who do not follow the Gym Rules and Code of Behavior, will need to have an adult accompany them on future visits.
Lockers/Showers
Lockers are available for your use while working out; there is no overnight storage. If you want to use a lock, you can sign one out, free of charge, from the Fitness Staff. Showers are also available. You must provide your own towel and shower supplies.
Pool Agreement
The Leech Lake Diabetes Fitness Program has an agreement with the Palace and Northern Lights Hotels for members of the Fitness Center to use their pools free of charge. You must obtain signed documents from the Fitness Staff to bring to the hotel. All rules and regulations of the hotel pool must be followed and will be enforced.
Medical Clearance
If the Health History Questionnaire identifies a medical condition, you must have a medical clearance to exercise. Please ask Fitness Staff for a blank copy of our medical clearance form for your medical provider to fill out.
