Accurate Home Care provides comprehensive home health care services to patients of all ages and care requirements. Accurate Home Care strives to be the best fit for patients who require round-the-clock, long-term care by adhering to its philosophy of partnering with health care organizations, listening to patients and their families and committing to only the most skilled, compassionate employees.
Depending on the service area our services include: Extended Hour Nursing, Skilled Nursing Visits, Therapy, HHA/PCA, Respite, Homemaking, and Mental Health Services.
Adult Day Services of Bemidji offers a reliable, caring and supportive environment where Seniors of varying abilities can go for socialization, healthy meals, health observation, and recreation. Seniors and their caregivers may choose Adult Day Services as a respite option or as a place for the Senior to be while the caregiver is at work all day. Support services are available for the caregivers as they transition their family member into higher degrees of service.
Adult Day Services promotes healthy aging with the focus to improve and enhance overall wellness.
Through a System of supportive services in one location the program conveniently assists people experiencing isolation due to physical and psychosocial demands. Attendance maintains and/or increases independence for older adults. Participants can continue to live at home and remain an active part of family and community.
Adult Day Services has handicap buses available to transport Seniors to and from their homes within a 10 mile radius of Bemidji. The buses are available for use by the public if the drivers participate in training, have appropriate licensure and insurance and are approved by ADS. Funding is provided by United Way and CADI. ADS clients are charged 50 cents per mile, billable to MA.
Committed to providing culturally sensitive care to the Native American population, both on and off the reservation. We take great pride in identifying the needs and concerns of our Native American cliental, the communities in which they live, and addressing those concerns with sensitivity and compassion. Services include: Personal Care Assistance, Homemaking Services, Home Care, RM Supervision and Respite Care.

Autumn Hills Assisted Living is a facility which offers assistance with all activities of daily living. We house up to 16 residents and are located in beautiful Bemidji, Minnesota. We are staffed 24 hours a day to provide your loved one with all of the personal care, supervision and security they may need. Our Assisted Living facility is able to provide a high level of care while increasing the personal satisfaction of the client at a reasonable cost to the consumer. Autumn Hills is licensed by the Minnesota Department of Health.

Sanford Baker Park Senior Living is located at our Sanford Lakeside Campus next to beautiful Lake Bemidji. Sanford Baker Park Senior Living provides housing subsidized by the United States Department of Housing and Urban Development. It provides housing for low-income seniors 62 and older. Residents usually pay 30% of their income toward rent.
Baker Park residents enjoy general maintenance, monthly social and leisure activities.

We focus on providing safe, happy, and healthy living environments for individuals with Intellectual/Developmental Disabilities, Mental Health Concerns, and Brain Injuries. Our services can be provided through one of the three main areas: Adult Foster Care, In-Home Support, and Semi-Independent Living Services.
The Veterans Services Division is part of the Health, Human, and Veteran Services Department. Our goal is to provide unparalleled customer service to the veterans of Cass County, with an emphasis on individual needs and personalized benefits counseling. Our customers have paid in advance for these services and deserve only the best. An individual’s right to privacy and confidentiality is always respected. The Clearwater Day Activity Center focuses on each individual’s needs to develop a specialized program in the areas of Home Living, Community Integration, Employment, and Independence. With one-on-one and group-centered training, those that attend the Clearwater DAC become more independent in daily living skills, are better able to contribute and interact within their community, and build lasting relationships through outside employment and group activities.
The participants at the Clearwater DAC enjoy a variety of activities from shopping, picnics at the park, bowling, and Special Olympics to family outings and real-life jobs within the area. Each activity promotes their individuality and the DAC’s mission to see these persons succeed on a daily basis with the skills they learn to thrive in every day life.

The Northern Cass Developmental Achievement Center and Developmental Achievement Center – Bemidji are 501 (c)3 non-profit, licensed day training and habilitation centers that offer vocational training and community job placements, as well as recreational and social opportunities, to adults with developmental disabilities.
Our services are funded through the Minnesota Department of Health, individual counties and school districts. We serve a total of 85 clients, from age 16 to age 78, with abilities ranging from problem-solving resourcefulness to steadfast determination and disabilities ranging from cerebral palsy to pervasive developmental disorders like Asperger’s syndrome and autism.

We strive to promote people’s potential by providing supportive employment opportunities, as well as social and recreational experiences, to adults with developmental disabilities.
DHHS provides direct consumer assistance regarding communication access; services to persons with combined hearing and vision loss; training and consultation; assistive technology demonstration labs; employment technology loans; mental health counseling; and telecommunications equipment.
The TED program provides telephone equipment to Minnesotans who have a hearing loss, speech disability or physical disability that limits their use of a standard telephone. The equipment is provided on a long term basis at no cost, as long as the individual/family meets the State’s requirements.

Disability Hub MN offers information about community resources related to people with disabilities, including housing, home and community-based services, and disability and cash benefits. This statewide service is a ‘no wrong door’ information and referral service for people with disabilities, their families, and support team. Disability Hub MN can be reached statewide by calling toll-free at 1-866-333-2466. The Hub is available Monday through Friday 8:30 a.m. to 5:00 p.m., to provide one-to-one assistance to help people learn about their options and connect with the supports and services they choose.
