Lighthouse provides services on a 24-hour or hourly basis. These services are designed around each individual and may include assistance with health and personal care, safety, nutrition, finances, activities of daily living, and training to independently perform these activities.
Lighthouse operates 5 homes that have 24/7 staffing to help fit the needs of individuals who have a disability. Lighthouse Lodge acknowledges the intrinsic value of each individual, regardless of position. We strive to acknowledge each person’s need for respect, privacy, meaningful work, recreation, community involvement and lasting friendships.
The primary goal of our program is to provide and facilitate the best possible living situation for each individual we serve. We achieve this by meeting their need for a home where emotional, spiritual, and physical needs are fully met
We provide services to Adults and Children with intellectual delays. We have CRS homes and ILS apartments located in Beltrami and Polk County of Minnesota.
Our services are designed for each person and will be based, as much as possible, on the lifestyles of other people of the individuals age, provide integration and independence, and provide training for future environments with as much independence and value as possible. It is also the program’s philosophy to provide the highest quality services available for the persons…and to offer them the opportunity to live and learn in the same setting and environment as a non-disabled persons, and to foster in each individuals potential for growth and development.
We base our programming on individual needs and wants.
We focus on providing safe, happy, and healthy living environments for individuals with Intellectual/Developmental Disabilities, Mental Health Concerns, and Brain Injuries. Our services can be provided through one of the three main areas: Adult Foster Care, In-Home Support, and Semi-Independent Living Services.

The Sanford Housing Program recognizes that housing is a basic right, and that following a personal service plan will be easier when housing is established. The program focuses on giving participants the opportunity to learn skills including successfully managing a lease and how to maintain permanent housing.
Our Focus: Choice of housing, separation of housing from services, decent, safe, and affordable housing, housing integration, access to housing, flexible, voluntary, and recovery-focused services.
Possible referrals to: Individual, family or group therapy, supported employment, adult rehabilitative mental health services, peer support services, substance use disorder services, medical services, GED services, case management, and medication management.
Housing Matters works with Long-Term Homeless individuals with diagnosed disabilities utilizing a Minnesota state funded program called Housing Support. Our agency works with private market landlords in the community to secure available housing options, broker and maintain relationships between landlords and participants, make payments of rent and utilities on a participant’s behalf, and to provide ongoing supports and basic need supplies.
Services
At Housing Matters we provide intensive case management services to build relationships that can foster individual growth. Implementing evidence based practices in our strategy to working with others is critical in achieving success. Some of the practices and philosophies that we implement in our work are; Motivational Interviewing techniques, recognizing the Stages of Change, utilizing a Harm Reduction approach, as well as understanding mental health, chemical health, and physical health barriers that can create serious difficulties for people in our community. We recognize that each person has a different set of skill sets and goals, but that everyone has a right to be provided with tools and resources to realize them. We believe that cyclical poverty, chemical health, and institutionalized racism have all played roles which have resulted in the current landscape of mental health, substance use, and its’ relation to homelessness.

We also can pay for a first or last month’s rent, security deposit if they are already homeless to get them housed. This grant has very little funding so we don’t use it as often. We have a Minnesota Assistance Council for Veterans (MACV) grant that helps veterans and their families with various needs.

Havenwood, where residents are cared for without regard to age, sex, race, color, creed, religion, handicap, or national origin. The Home is licensed by the State of Minnesota, certified for Medicare and Medicaid, and complies with all standards mandated by regulating agencies.
Havenwood Care Center (formerly Beltrami County Nursing Home) has a long history of caring for the elderly. Originally, long-term care services were provided at the Lake Julia farm, 15 miles north of Bemidji. From 1917 until 1968, that facility was one of the few in the region dedicated to the ill and aged who required long-term care. In 1968, the old home was retired and a new 85 bed facility opened at our current location. In 1974, a 48 bed addition extended capacity to 133 beds. We currently utilize 90 beds.
Although the site has changed, the same dedication and spirit of love and understanding exists. We continue our pledge to maintain the highest possible standards of nursing care and to provide social, recreational, intellectual, spiritual, physical and re-motivational activities specifically designed to meet individual needs.
Havenwood is a place where the joys, frustrations, and excitement of living are shared by residents and staff.
