Baker Park is a residence for elderly individuals located in Bemidji, Minnesota. The facility is situated in a picturesque lakeside setting within a short distance of the downtown area with shopping, library, transportation, recreational facilities, and many churches nearby. It has easy bus access to the hospital, medical clinic, and shopping mall.
Eligibility Criteria
Age – 62 and older
Financial – HUD guidelines impose income limits and require that tenants generally pay 30 percent of adjusted gross income toward their rent. Income limits change. Please check on current limits.
Community Living at Baker Park – A person may be considered capable of living in a community setting at Baker Park if he/she is able to live up to the terms of the lease and can handle the activities of daily living.
Lease – Tenants are required to sign a one-year lease upon move-in.
Autumn Hills Assisted Living is a facility which offers assistance with all activities of daily living. We house up to 16 residents and are located in beautiful Bemidji, Minnesota. We are staffed 24 hours a day to provide your loved one with all of the personal care, supervision and security they may need. Our Assisted Living facility is able to provide a high level of care while increasing the personal satisfaction of the client at a reasonable cost to the consumer. Autumn Hills is licensed by the Minnesota Department of Health.
Autumn Hills Provides:
~ Home-like living with the assistance to meet the special needs of the elderly or disabled
~ Companionship, safety and supervision, and personal services
~ Daily cost is based on an individuals care needs as determined by the RN assessment
~ Pay sources accepted include: private, elderly waiver, Alternative Care, CADI, and long-term care insurance.

Orthopaedics
We’ve treated orthopaedic conditions since the founding of our first hospital in 1922. With a large full-time staff of pediatric orthopaedic surgeons in the United States, as well as a comprehensive team of physical, occupational, speech and other therapists, we are able to provide customized care to each child.
Burn care
Since first treating burn survivors in the 1960s, we’ve pioneered some of the most significant advancements in burn care. We provide advanced care for burn injuries and related scarring, along with physical rehabilitation and emotional support.
Spinal cord injury
We were the first health care system in the country to develop spinal cord injury rehabilitation centers designed specifically for children. Our groundbreaking research has led to innovative treatment techniques, providing care that helps children lead a full, active and healthy life.
Cleft lip and palate
Our cleft lip and palate reconstruction program gives kids a reason to smile. Each treatment plan is crafted uniquely for the child by a team of experts working together to improve a child’s eating, breathing, communication, self-esteem and appearance.
Lakes MediVan offers non-emergency, door through door transportation by handicap van with wheelchair and stretcher capability. The vans travel 24 hours a day, 7 days a week as far as needed, including out of state. Medi-Van® serves 200 mile radius of Detroit Lakes. The mission of the Disability Services Office is to improve the educational development of students with disbilities, enhance understanding and support within the campus community, while providing opportunities for student growth in multicultural understanding, civic engagement and environmental stewardship. Students will be better prepared to enter their communities as contributing and supporting members with an understanding of the diverse needs of their community.
The Ombudsman is an independent governmental official who recieves complaints against government (and government regulated) agencies and/or its officials, who investigates, and who if the complaints are justified, takes action to remedy the complaints. The Office of Ombudsman Mental Health and Developmental Disabilities (OMHDD) provides resources in regarding: 1) Client Services, 2) Medical Review, and 3) Civil Commitment Training.

Health and personal care services are available, including medication administration, health monitoring, incontinence care, bathing and foot care.

Community Services for the Deaf provides an array of communications tools and technology that enables functional equivalency for deaf and hard of hearing individuals.
Long Term Care: Rehabilitation and skilled care (sometimes referred to as nursing home or skilled nursing care) refers to care that is provided for residents who need the assistance of rehabilitative and/or licensed nursing staff.
Rehibition Department: An in-house therapy department provides inpatient and outpatient occupational and physical therapy. Inpatient speech therapy is also available. Contact the Therapy Department to review eligibility options.

Options is a non-profit organization committed to assisting individuals with disabilities to live independently in the communities of their choice and to eliminate barriers of attitude, architecture and communication from the environment.
Options: Interstate Resource Center for Independent Living (IRCIL) is a Resource Center for Independent Living. We are a private, nonprofit center that provides services to maximize the independence of individuals with disabilities. We are funded by various State and Federal grants, as well as private donations.
We are a consumer-controlled, community-based, cross-disability, non-residential center that is designed and operated within a local community by individuals with disabilities and provide an array of independent living services . Our services are free of charge to individuals requesting them. Since October 1986, Options: IRCIL has worked to make our service area a better place to live, work and play for people with disabilities.
We serve counties in Northeast North Dakota and Northwest Minnesota.
North Dakota counties include: Benson, Cavalier, Grand Forks, Griggs, Nelson, Pembina, Ramsey, Steele, Towner, Traill, Walsh, Wells, Eddy and Foster.
