Advanced care planning provides information about your care preferences to health care providers, family members, and others involved in such decisions.

Native American Family Caregiver Support Program Requirements
Native American Family Caregiver Support Program can serve two types of unpaid family caregivers:
- Family caregivers: An adult family member, or another individual, who is an unpaid informal provider of in-home and community care to an older individual or to an individual with Alzheimer’s disease or a related disorder with neurological and organic brain dysfunction.
- Grandparents or older individuals who are relative caregivers: A grandparent or step grandparent of a child, or relative of a child by blood marriage, or adoption, who is 55 years of age or older and
- lives with the child;
- is the primary caregiver of the child because the biological or adoptive parents are unable or unwilling to serve as the primary caregiver of the child; and
- has a legal relationship to the child, as such legal custody or guardianship, or is raising the child informally.
The five required services are:
- Information to caregivers about available services.
- Assistance to caregivers in gaining access to services.
- Individual counseling, organization of support groups, and caregivers training to assist the caregivers in the area of health, nutrition, and financial literacy, and in making decisions and solving problems relating to their caregiving roles.
- Respite care to enable caregivers of frail elders to be temporarily and intermittently relieved from their cargiving responsibilities.
- Supplemental services, on a limited basis, to caregivers of frail elders to complement the care provided by caregivers.

CHILD CARE ASSISTANCE PROGRAM (CCAP)
Child care assistance is available to parents who are receiving assistance under the Minnesota Family Investment Program (MFIP) and parents whose income is at or below 175% of the Federal Poverty guideline and are in an approved activity. It is important for parents to know their child is well-cared for while they are at work, looking for work, obtaining training or going to school in order to secure employment. Application is required to have eligibility determined. CCAP may not cover all child care costs. Parental amounts depend on the family’s income, the number of people in the family, and how much the child care provider charges. The goal is to assist the family to maintain employment and gain self-sufficiency.
CHILD SUPPORT
Every child needs financial and emotional support and every child has the right to support from both parents. Minnesota’s child support program benefits children by enforcing parental responsibility for child support.
Either parent of a child may apply or qualify for certain support services from the Child Support Unit. Depending on the circumstances associated with the case, there may be fees assessed for collection unless the applicant receives public assistance (Minnesota Family Investment Program, Medical Assistance, MinnesotaCare, or Childcare Assistance.)

Family Safety Network (FSN) is a non-profit organization providing advocacy, support, and referral services to domestic violence victims and their families since 1993. Family Safety Network was founded by a group of concerned citizens who saw a need for this service in the community. On any given day in the state of Minnesota, 1,975 adults and children are victims of domestic violence and 47.7% of those happen in rural Minnesota. Over the past 12 months we have provided 13,133 services to clients throughout our area townships. We have assisted 703 families: including 60 homeless clients and 151 clients needing emergency shelter. We provided emergency financial assistance to 516 victims and their children.
Last year we shared that our most pressing emerging need would be shelter. Today, we are happy to share that we were able to create a program using the Hotel/Motel Voucher Shelter System. There are no Domestic Violence Shelters in our service area; this program allows us to provide emergency shelter at the time of crisis and our programs allow us to create a person-centered advocacy plan to move families to housing stability and break the cycle of abuse.
