Evergreen Youth & Family Services is a community-based organization committed to strengthening youth, preserving families, and building relationships with an emphasis on those living in Northern Minnesota. Evergreen serves youth experiencing homelessness, runaways, and families in crisis by providing housing resources, youth crisis shelter, food & clothing, hot lunches 5 days/week, referrals, family support & parent coaching, mental health support, independent living skills, referrals to other community resources and more.
All services are open to adults 18 years old or older who have a long-term severe and persistent mental illness. Services include Adult Rehabilitation, Mental Health Services (ARMHS). Outreach, diagnostic assessments, medication reviews with a psychiatrist, after-hours emergency help, nurse visits, community/public education, phone contact, social and recreational activities in a drop-in center and off-site client employment help, and the REACH family support group.
ABC offers mental health services and is a Licensed Rule 31 Facility able to work with adolescents and adults with co-occuring disorders. (Mental health and chemical dependency combined) with public and private insurances. ABC also provides intensive outpatient services and intensive outpatient program groups held in the evening hours. Regional Navigator for sexually exploited and trafficked individuals
Operated by non-profit, Canvas Health, Crisis Text Line allows users to communicate with trained counselors via confidential text messaging. Counselors help people of all ages and walks of life get to an emotionally and physically safe place and then direct them to community resources that can provide longer-term support. They serve many counties and all Tribal Nations located in Minnesota. Text “MN” to 741741.
Assertive Community Treatment (ACT) is an evidence based model for providing comprehensive community-based treatment to person with severe and persistent mental illness. The Headwaters ACT Team is a multidisciplinary group organized as mobile mental health service providers.
ACT is a way of delivering a full range of services to people who have been diagnosed with a serious mental illness.
Elements of ACT:
-With ACT, consumers are hospitalized less often and have more stable housing
-Team members are involved and familiar with all consumers
-The team works one on one with individual clients and families
-Cultural respect and guidance is offered by a Cultural Coordinator
With ACT, consumers get help taking care of their basic needs – taking medications, cooking and many other individual needs. The ACT team helps consumers find housing, apply for food stamps, go back to school, or get a job.
How does ACT work?
A Team Approach: An Advanced Practice Registered Nurse (APRN), RN, licensed mental health professionals, mental health practitioners, an employment specialist, a peer support specialist, a cultural coordinator, and a substance abuse specialist join together on ACT teams to give consumers ongoing, individualized care.
Services Provided Where They are Needed: Consumers receive ACT services in their homes, where they work, and in other settings in the community where problems occur or where support is needed.
Personalized Care: ACT teams work with relatively small numbers of people, those in greatest need of intensive services to live independently in the community.
Time-Unlimited Support: ACT teams give consumers services and support they need for as long as they need them.
Continuous Care: ACT team members work regularly with frequency of services that can range from several contacts a day to once a week or less depending on the need of the client.
Flexible Care: ACT teams fit their schedules around the needs of the consumers.
Comprehensive Care: ACT teams provide an array of services to help meet consumer needs, including case management, nursing, representative payor services, vocational, medication management, alcohol and drug services and skill building.
The Sanford Housing Program recognizes that housing is a basic right, and that following a personal service plan will be easier when housing is established. The program focuses on giving participants the opportunity to learn skills including successfully managing a lease and how to maintain permanent housing.
Our Focus: Choice of housing, separation of housing from services, decent, safe, and affordable housing, housing integration, access to housing, flexible, voluntary, and recovery-focused services.
Possible referrals to: Individual, family or group therapy, supported employment, adult rehabilitative mental health services, peer support services, substance use disorder services, medical services, GED services, case management, and medication management.
