Community Resource Connections (CRC) is a nonprofit membership organization serving Beltrami, Cass, Clearwater and Hubbard Counties.
MNsure Navigators: Application for Medical Assistance, Minnesota Care, and Qualified Health Plans offered through the state of Minnesota’s health exchange. CRC staff provide computer and internet access, and question by question guidance. No cost to you. Appointments required.
Service Access: Assistance finding the resources you need. CRC staff will educate you about your service options, assist you with service applications, and advocate for you when the system doesn’t work as you need it to work. No cost to you.
Nutrition Assistance Program for Seniors (NAPS): CRC’s Blackduck office site takes applications and serves as a dissemination point for food commodities provided through North Country Food Bank. Eligibility is based on age (60+) and income (call for guidelines). Free monthly food boxes containing a $40 value of non-perishable food items.
Community Service Agency Membership: CRC works with community service agencies to organize and publish a hard copy resource directory as well as this electronic version. Other member services include: facilitation of networking meetings, cooperative marketing events, nonprofit start up guidance, and program support.

Evergreen Youth & Family Services is a community-based organization committed to strengthening youth, preserving families, and building relationships with an emphasis on those living in Northern Minnesota. Evergreen serves youth experiencing homelessness, runaways, and families in crisis by providing housing resources, youth crisis shelter, food & clothing, hot lunches 5 days/week, referrals, family support & parent coaching, mental health support, independent living skills, referrals to other community resources and more.
NWICDC is located in Bemidji, MN and provides services to more than 1,000 people and families annually. We prioritize the 60-mile radius, which includes the Red Lake Nation, White Earth Nation, the Leech Lake Band of Ojibwe and the administrative headquarters of the Minnesota Chippewa Tribe. This region continuously ranks the lowest in economic and social outcome indicators in Minnesota, and . The Northland Area Family Services Center was created as a unique way to give rural access to county and educational services, and to meet community needs through innovative programming. Incorporated in 1996 as a 501C3, we serve the Northland Community School District and parts of Leech Lake Reservation. This is a population of 4,474 people (5.26 persons per square mile). In our service area 20% of households make less than $20,000 and 11.9% or 536 people in our community live below the poverty line.
Anishinabe Legal Services is a Legal Services Corporation (LSC) grantee and independent 501(c)(3) non-profit that provides free legal assistance to low-income individuals living on or near the Leech Lake, Red Lake, & White Earth Reservations in Northwestern Minnesota.
ALS represents clients before tribal courts, state courts, and agencies to help them protect their rights. Anishinabe Legal Services has offices in Leech Lake, Red Lake, and White Earth.
Residents of Leech Lake, White Earth, and Red Lake reservations whose income and assets fall within the financial eligibility guidelines. Services are also provided to residents of Leech Lake, White Earth, Grand Portage, and Bois Forte who are 60 and older regardless of income.

Therapeutic transitional living program, providing support to adolescent girls who have been sexually exploited and/or trafficked. Serves girls ages 16-21. The Red Lake Comprehensive Health provides the management, leadership and direction needed to provide quality health services to the Red Lake people. Our main goal is to elevate the health and health care of our Red Lake Tribal members and their descendants to the highest possible level.
Emergency Medical Services
Two Ambulance Services, one in Red Lake and one in Ponemah provide prompt emergency medical care for sudden injury or acute illness and immediate transportation to a medical facility where further care can be rendered to eliminate severe complications from untreated trauma, twenty-four hours a day/seven days a week.
The Ambulance Service are licensed by the State of Minnesota as a Basic Life Support Service.
The Ambulance staff are required to be certified State Emergency Medical Technicians in order to maintain our Basic Life Support (BLS) License. The majority of our Emergency Medical Technicians are Nationally Registered Emergency Medical Technicians.
The Emergency Medical Technicians provide in-services and training to tribal members that want to learn Cardiopulmonary Resuscitation, First Aid and Emergency Medical Technician skills. We are licensed by the State of Minnesota as an official training program for Emergency Medical Technicians, CPR & First Aid.

Medical Transportation Services
Medical Transportation is provided for the people to the Red Lake Hospital, Ponemah Clinic and medical facilities off the reservation.
Patients are transported to Minneapolis, Rochester, Duluth, St. Cloud, Minnesota, Fargo, Grand Forks, North Dakota and other towns as directed by the Medical Staff. Medical transportation is provided to the Red Lake Hospital, Ponemah Clinic and to the Bemidji Hospital and Clinics on a daily basis.

Community Health Nursing Services
Community Health Nursing Services provides health services to meet the assessed health needs of individuals and families in our four communities. Services are provided in hospital, clinic, home, school and community settings.
Community Health Nursing Services are provided within the context of total health care, including physical, social, psychosocial, environmental developmental, parenting education, nutritional economics and functional health. Nursing is provided at three levels of prevention: The primary prevention level focuses on health promotion and prevention against diseases, the secondary prevention level focuses on early diagnosis and prompt intervention in the disease process, and the third prevention level focuses on the use of rehabilitation activities to prevent further complications and to restore as much as possible optimal functioning. Language, communications and cultural considerations that influence care are considered. Individuals, families, and communities are encouraged to be active participants in determining their needs and reaching health outcome goals.

Nutrition services provides and promotes sound nutritional practices for individuals, families and groups in the Red Lake Hospital and Health Care Facility, Ponemah Health Center, Elderly Nutrition sites, Maternal Child Day Care Center and the Headstart Programs; preventing potential health problems; and providing clinical nutritional and diabetic services for patients with existing medical problems.

Women, Infants and Children (WIC) helps families eat well and stay healthy. The WIC program provides nutrition education, healthy foods and health services. WIC does nutrition assessments, and provides one-on-one counseling about food, nutrition, and breastfeeding to help you feed your family in a healthy way. WIC gives you vouchers to buy healthy foods. WIC helps you with referrals to other health and social services. To qualify for WIC you need to have a nutritional and/or medical need, and meet WIC income guidelines.

The Sanitarian assists all communities in improving the public health. Environmental Health Services identifies and helps correct environmental conditions that are detrimental to the people’s health status. Security Services
Security services are provided for patients, staff and property at the Red Lake Hospital and Health Care Facility and the Extended Care Center. Security services work cooperatively with the local law enforcement agency for added safety.

Mental Health/Social Services
The Comprehensive Health Services provides two Independent Licensed Social Workers, Social Workers and School Wellness Counselors to assist individuals, patients, families in the schools and community promoting positive behavior, health promotion and disease prevention through direct assistance, counseling and coordination between home, school, and community and health facility.
The staff provide outreach services and function as a community resource in dealing with maladaptive behavior, and provide intervention in seeking out and eradicating conditions that caused or contributed to mental disorders.
The School Wellness Counselors provide counseling in the schools and communities.
The staff work with individuals who are having thoughts of hurting themselves, are having suicidal ideation, self-injurious behavior and behavioral emergencies.

Comprehensive Health Provides $25,000 a year to purchase dentures and partials for tribal members and their descendants at the Red Lake Hospital only.

Optometry provides comprehensive optometric services including vision assessment, eye evaluation, retinal assessment, frame adjustment and repair, eye injury treatment, eye disease treatment, and contact lens fitting.

Special Diabetes Program for Indians – SDPI
The Red Lake Diabetes Program provides excellent care and education to all patients with diabetes, prediabetes, and those at risk for diabetes. Patients are seen as outpatients, inpatients and in the community. We provide one on one diabetes education, nutrition education, physical activity information, community diabetes screenings, youth wellness summer camps, diabetic socks and testing supplies given, current information on diabetes is provided to patients and diabetes related information is submitted to the Tribal Newsletter on a quarterly basis.

Nursing Home Services are provided at the Jourdain/Perpich Extended Care Center which is attached to the Red Lake Hospital and Health Care Facility. We have 47 beds available to serve our elders and handicapped individuals. All residents served are federally recognized Indians or descendants of federally recognized tribes.

Adult Day Services of Bemidji offers a reliable, caring and supportive environment where Seniors of varying abilities can go for socialization, healthy meals, health observation, and recreation. Seniors and their caregivers may choose Adult Day Services as a respite option or as a place for the Senior to be while the caregiver is at work all day. Support services are available for the caregivers as they transition their family member into higher degrees of service.
Adult Day Services has handicap buses available to transport Seniors to and from their homes within a 10 mile radius of Bemidji. The buses are available for use by the public if the drivers participate in training, have appropriate licensure and insurance and are approved by ADS. Funding is provided by United Way and CADI. ADS clients are charged 50 cents per mile, billable to MA.

The Bemidji Senior Center, officially known as The Beltrami County Senior Citizen Council on Aging, is located in beautiful downtown Bemidji at 216 Third Street NW. Open Monday-Friday, 9:00 AM to 4:00 PM the building comes alive early as the Senior Creations Gift Shop opens for business and seniors come in for the various programs offered. The Senior Center is a place where active adults can receive nutritious meals, socialization, exercise, tax assistance, a driver safety refresher course, informative lectures, creative activities, volunteer opportunities, and a sense of belonging.
LSS Nutrition Program: Lunch every day Monday – Friday at noon. You MUST call at least one day in advance to reserve your meal. Call between 9:00am – 1:00pm to make a reservation. Suggested contribution for seniors age 60+ is $4.00. People under age 60 is $8.00. Gift certificates and meal tickets are available at the site. Delivery to your home is also available (Meals on Wheels).
Tax Assistance: AARP tax preparation counselors provide assistance for seniors and low-income individuals Mondays, Thursdays and Fridays from 9:15am – 3:15pm starting the end of January through mid-April. Help with property tax and rent refunds are available the second Monday of each month from May – September from 9:00am – 12:00pm. Assistance is provided on a first come, first serve basis. This is a free service, however, donations to the center are appreciated.

The American Cancer Society offers free comprehensive cancer information with day to day emotional support, and a 24 hour information phone line. Free temporary lodging in the Twin Cities or Rochester for families who must travel outside their community for care. The ACS also offers classes, access to a Cancer Network, and special products for women with cancer.
Our mission is to provide a quality and creative approach to individual and community based exercise programming to ultimately control and prevent diabetes in our community.
Also note: person(s) do not need to have Diabetes to use the fitness center.

Gym Rules
- Diabetic clients have first priority on the equipment
- Use equipment at your own risk
- Sanitize equipment after use
- Return all equipment after use and re-rack your weights
- No horseplay allowed
- Appropriate footwear required while using equipment and lifting weights
- Know your limits; use a spotter as needed
- No gym bags, coats, etc. on gym floor; use lockers and coat rack
- Not responsible for lost or stolen items; locks available upon request
- No food allowed on the gym floor
- All liquids must be in containers with lids
- For safety reasons, no children under 10 allowed
- Be considerate of others
Code of Behavior
All fitness center participants are required to conduct themselves in an appropriate manner. Inappropriate language (swearing, derogatory terms, etc.), inappropriate behavior and harassment are not allowed on the premises. Proper use of the equipment is required. If you do not know how to operate the machines, the fitness staff will be happy to assist you. There is a zero tolerance policy when it comes to horseplay in the fitness center and on the machines. You will be asked to leave if you do not follow the Code of Behavior and Gym Rules!
Age Restrictions
- Children 13 and under must have an adult with to supervise them during each visit.
- Children 11 and under must have a medical clearance on file.
- For safety reasons, no children under 10 allowed.
- Young adults 14-17, who do not follow the Gym Rules and Code of Behavior, will need to have an adult accompany them on future visits.
Lockers/Showers
Lockers are available for your use while working out; there is no overnight storage. If you want to use a lock, you can sign one out, free of charge, from the Fitness Staff. Showers are also available. You must provide your own towel and shower supplies.
Pool Agreement
The Leech Lake Diabetes Fitness Program has an agreement with the Palace and Northern Lights Hotels for members of the Fitness Center to use their pools free of charge. You must obtain signed documents from the Fitness Staff to bring to the hotel. All rules and regulations of the hotel pool must be followed and will be enforced.
Medical Clearance
If the Health History Questionnaire identifies a medical condition, you must have a medical clearance to exercise. Please ask Fitness Staff for a blank copy of our medical clearance form for your medical provider to fill out.
